No, you do not need to have money in your PayPal account in order to make a PayPal payment. You simply need to have your bank account, credit or debit card details stored on your PayPal account, which can then be used to fund your payments. Each time you want to pay with PayPal, you can select whichever card or bank account you have stored.
If you get paid for an item you sold on eBay, or if someone sends you money, you will have a balance in your PayPal account. This can then be used to fund your payments; however it's not necessary to have a balance in order to use PayPal.
